Guidelines for preparing Program Portfolio

(Includes Documentation for Program Assessment and Program Review)

 

Each program is responsible for keeping the program documentation, which includes the assessment documentation (plan, assessment results, and reports) and additional documentation essential in preparing the 5-year (or 3-year) program reviews required by the HEPC, or portfolios needed for the evaluation visits of the accrediting agencies.  To assist in this effort of collecting and keeping this documentation, a Program Portfolio, one for each program, has been created. These Portfolios include, but are not limited to, the following documents:

 

  1. Curriculum Data (curriculum, catalog descriptions and prerequisites, recent curriculum changes, and articulation agreements. Administration will keep record of this data and provide them to you at your request.)

  2. Program Assessment Plan (The generic program assessment matrix is provided. It is responsibility of the program directors and program faculty/instructors to keep it updated.)

  3. Course Syllabus and Course Assessment Documentation (Responsibility of faculty/instructors, coordinated by program directors.)

  4. Annual Program Assessment Report and Improvement Plan (Responsibility of program directors and program faculty/instructors.)

  5. Faculty Data (degrees, certificates, teaching area, development, publications/presentations. Responsibility of faculty and instructors too keep their own updated data. Faculty Data form is provided. Transcripts of the faculty are archived in the President's Office. Also, adjunct faculty data forms are being archived by the President's Office. )

  6. Annual Course Enrollment Data (Course number, title, schedule, instructor teaching the course, enrollment, and mode of delivery, and faculty load report. Data archived and provided on request by the Dean of Academic Affairs)

 

Most of this documentation will be archived in the electronic form and when required (for program review, NCA or other accrediting agency visits) it can be easily converted into a portfolio. There will be secretarial help available for putting together the initial program portfolios.
